We are delighted to serve you by sharing knowledge of yoga. The following information will help you get the most out of your yoga classes. Please read and fill up the form below.
Awareness is fundamental to the practice of yoga. It is your responsibility as a student to monitor each activity and determine whether it is appropriate for you to participate. You are primarily responsible for your safety and well-being. Please inform us/teachers of any health
condition that could be affected by your practice of yoga. If you are unsure about a condition, feel free to speak with us/ teachers.
The following guidelines will help you reduce your risk of harm or injury.
- Listen to and follow the instruction carefully.
- Breathe smoothly and continuously as you move and stretch.
- Do not hold your breath or stain to attain any position.
- Work gently, respecting your body’s abilities and limitations.
- Menstruating women should not practice inverted and some twisting postures.
- Pregnant ladies must obtain permission to attend regular classes.
In filling up the below, I voluntarily assume a certain risk of harm or injury by choosing to participate in the yoga classes. I agree to take full responsibility not to exceed my personal physical limits in the practice of yoga, and for any injury I might suffer during my participation in classes and activities at the YOGA MITRA studio. It is my responsibility to ascertain that there is no medical reason to prevent my participation in class or any studio activities. Should I sustain harm or injury, I agree that neither me, nor my heirs, assigns, nor legal representatives will sue or file a claim of any kind whatsoever against the YOGA MITRA, its owner, teachers, employees, participants, lessors, lessees or owners of the premises for damages or costs resulting from my participation. I, my heirs and legal representatives, forever release, waive, discharge and covenant not to sue the YOGA MITRA for injury or death as the result of professional negligence or other acts.
I understand there may be physical touch and adjustments involved in the class that are specific to me, observed as necessary by the teacher, and that these physical assists and adjustments are solely designed to assist me in the yoga postures. If in the course of a physical adjustment or a physical assist I withdraw consent, then it is MY responsibility to say “STOP” or otherwise communicate clearly and audibly that consent has been withdrawn .
If the participant is a minor: A parent or guardian needs to sign this release form in additional to the minor. Through filling up the release form below, the minor has my consent to participate in the class and that the release of liability set forth above is applicable above is applicable to such minor. I further provide my consent for the YOGA MITRA to seek emergency treatment for the minor if necessary. I agree to accept financial responsibility for the costs related to this emergency treatment.
